IVF journal prompts for every stage

IVF moves fast and asks a lot of you. Writing through it — even a few sentences — can help you notice what you are feeling, remember what matters, and carry your story forward. Here are gentle prompts for each stage of the journey.

Calendar & Protocol

The plan lands in your inbox.

  • What protocol did your clinic put you on, and how was it explained?
  • What questions do you still want to ask your care team?
  • How are you feeling about the calendar in front of you?
  • Who knows about this cycle, and who are you keeping it from for now?

Hormones & Injections

Stims begin.

  • First injection — what did it feel like, physically and emotionally?
  • Track today's meds, doses, and timing.
  • Side effects showing up in your body or mood?
  • One small kindness you gave yourself today.
  • Monitoring appointment notes: follicle counts, E2, lining.

Egg Retrieval

Trigger, retrieval, recovery.

  • Trigger shot: date, time, how it felt.
  • The morning of retrieval — what do you want to remember?
  • How many eggs were retrieved? How did that number land for you?
  • Recovery: pain, bloating, rest, food, comfort.
  • A note to yourself for the next few days of waiting.

Embryo Development

Day 1 through blastocyst.

  • Fertilization report: how many made it to day 1?
  • Day 3 update — cell counts and grades.
  • Day 5 / 6 / 7 blastocyst report.
  • What is it like to wait for these calls?
  • PGT-A results, if you tested — and how you're holding them.

Selecting an Embryo

Choosing which one to transfer.

  • Which embryo are you transferring, and why this one?
  • How did you and your care team make this decision?
  • What are you hoping for, and what are you afraid of naming?
  • A message to this embryo before transfer day.

Embryo Transfer

Transfer day.

  • What did the morning of transfer look like?
  • Who was with you? What did you wear, eat, listen to?
  • The image from the transfer — describe what you saw.
  • First words you said to your embryo after transfer.
  • Instructions from your clinic for the days ahead.

The Two Week Wait

Between transfer and beta.

  • Day-by-day symptoms (or lack of them).
  • How are you distracting, coping, holding on?
  • Any at-home tests? How did that feel?
  • Beta day: number, nurse's voice, your reaction.
  • Second beta and doubling — what did they say?

First Ultrasound

Gestational sac.

  • Date, weeks + days, what was measured.
  • What did you see on the screen?
  • What did your doctor say — and what did you feel?

Heartbeat Ultrasound

First flicker.

  • Heart rate, measurements, gestational age.
  • Describe the moment you saw the heartbeat.
  • Who did you tell first?

Follow-up Ultrasounds

Weekly monitoring at the clinic.

  • Log each scan: date, weeks, measurements, notes.
  • How is your body feeling week to week?
  • Meds you're still on (progesterone, estrogen, etc.).

Graduation to OB-GYN

Leaving the fertility clinic.

  • Last day at the clinic — what do you want to remember?
  • A thank you (or a goodbye) to your care team.
  • First OB appointment: date, provider, first impressions.
  • Looking back at the version of you who started this journal — what would you tell her?

In Loving Memory

For the cycles that do not end the way we hoped.

  • Did this embryo or baby have a name? What did it mean to you?
  • What special ways will you remember this life?
  • How has this experience changed you, and your thoughts about IVF?
  • Do you plan to take a break before another transfer?
  • What compassionate care can you give yourself right now?
  • What do you most need in this moment?
  • Anything you'd like to say to your baby.
